In my experience, I have found that most people don't even have their pianos tuned yearly. Delaying a tuning can not only make the piano more difficult to tune (often requiring more than one visit to bring the piano back up to pitch), but it can also cause irreparable damage to the tuning pins and pin block!
The inside of a piano (especially an upright) is usually dark and cool and seldom sees the light of day. Moths, spiders, and other critters love to make their homes here. There is a LOT of felt inside a piano. Moths love to eat that felt, and missing felt can mean internal wear and damage as the wood and metal parts rub against each other.
